SCHOLARSHIP PROGRAM FUND FORM

Eligibility Requirements:

Candidate must be 21 years of age or older.
Candidate must be life time member of SLKPS or become life time member.
Candidate must be a member of LKPS by birth (at least one parent must be a part of LKPS).
Candidate must be a citizen or permanent resident of the United States or Canada.
Candidate must be present at the election site on the day of elections unless there is emergent issue.

Timeline:

Completed candidacy form due by July 31, 2017
Elections will be held during the upcoming SLKPS Convention if required.
The serving term will be two years or more
